I think he can look however he wants to look. He doesn't actually have a physical body. He's a demon-king in hell.

thats kind of what I thought, story wise, with the Marvel Universe anyway,
artistically, should I chalk it up to the same thing?

I would, personally.

Is it that he can look however he wants, or is it he can look how people perceive him?

I don't know. Either is possible. From a practical perspective, they're equivalent. The artist has license to design Mephisto's look however the artist likes.

I personally doubt Mephisto would leave the visuals entirely up to the living creature's subconscious. Some living creatures may have no frame of reference at all from which to make an image. Thor, for example, or the Silver Surfer, neither of whom would have a Judaeo-Christian devil image in their subconscious, might simply perceive Mephisto as a void. Yet both tended to see him pretty much identically, and the real reason for that was, in both cases the artist was the great John Buscema.

Also, sometimes he looks different inside and outside of his realm. Most of his power comes from his realm.

Supposedly Galactus looks how people perceive him. I've always wondered if that means Galactus is black to black people.

Reverend Meteor (one of the dumb things I wonder about)

Because he's the devil. (or a reasonable facsimile)

And aside from that time in Infinity Crusade has he ever really looked all that different from the way he usually looks?

Depends on the black person, probably. The diaspora probably has African-Americans seeing him White.

My favourite look of his was the GR/Wolverine/Punisher look (also in Daredevil circa #270) with the freaky hair and long snaky tongue. ♥JRJR

It was around that time that Marvel gave Satannish a new look, as well.
