IIRC, after Spidey got the symbiote off himself with Reed Richard's help, we saw it later break out of his FF base's glass cage somehow, then it's next appearance was Web of Spider-Man #1, right? Also, which Spider-Man title/issue-# was his escape from the FF building?

I know that it definitely happened during the De Falco/Frenz run. It was right around #258, maybe #260 or so. I THINK that it made an appearance in Spectacular before Web Of as well. I hope that helps. I think that you can get the Black Costume saga trade.

The symbiote was captured in ASM 258 and was shown in almost all issues afterward leading up to Web #1(ASM 259-261, Spec 97-100, MTU 148) and also Fantastic Four 274. I believe it escapes in ASM 261 and is shown bonding with random New Yorkers in Spec 99 and 100 before it finds Spidey in Web 1.

Wasn't Doom's ward Kristoff the one that released the symbiote from the Fantastic Four's base?

I always found it weird that Kristoff's role in unleashing the symbiote upon the world was never really referenced again.

I didn't remember that, but you're right. It was Kristoff.
Here's an article that deals with this. I misremembered the scene as the symbiote somehow mesmerizing one of Reed's drones.

Well for some reason I think when the story of the symbiote's escape was retold from then on (aside from ASM #261) the bit about Kristoff's device freeing the symbiote is omitted.

Considering how many people Venom and Carnage have killed I think Kristoff needs to be taken to task for allowing this evil out into the world. A minor character did something that's changed the course of the Marvel Universe and for decades has not been held accountable.
