My guess would be W'Kabi ruled Wakanda with the understanding that he wasn't and would never be king and the Wakandan government would appoint a new king at a later date if T'Challa did not return. W'Kabi was T'Challa's second in command and had the highest military position in Wakanda. I don't think he has the royal blood to be declared king but I think someone in W'Kabi's position could be a regent until a new king was declared.
Or T'Challa's step-mother Ramonda...but I don't think she was in Wakanda at the time. And I don't think they would invite her to be queen or regent since she was from South Africa. W'Kabi was Wakandan and while not a royal he had IMO the most important job outside of being king as he was Wakanda's head of security.
I imagine some of T'Challa's cousins would have made a play for the throne. Surprised that didn't happen.
Of course back then, she wasn't even thought of yet
