The death of Jean's family. It sounds like it happened in an X-Factor issue or storyline (Probably during Simonson's run.).
To answer the question, the X-universe has definitely become too convoluted in regards to characters like Psylocke, Cable, Rachel Summers, etc. I would also remove Vulcan, Wolverine's son, and Emma Frost becoming a villain again. Other things would be Longshot (He might not be a bad character, he just doesn't fit in on books. I don't think that he was meant to.), Shatterstar's origin and Nightcrawler's demonic origin, and Professor X covering up the mission via Deadly Genesis.I also liked Magneto being Quicksilver and Scarlet Witch's father, so the should have kept that. And, oh yeah, all of that confusion about Xorn (After Morrison left.).

Rachel is invited to the Grey's house for a family reunion, and in 24 seconds, the Shi'ar Death Commandos destroy every man, woman, and child.

And the story is topped off by Mama Grey's ghost spewing hate towards mutants. But Jean comes and takes them all to the White Hot Room. Though that might have happened in one of the next two issues of the arc.

It's a very emotional story as it should be, and kudos to Claremont for that, but... O... M... G!

Oh... they also give Rachel a huge, genetic, tribal, Phoenix tramp stamp that served as a universal locator.
