IN THE STORY "Threat of the Tornado Tyrant", from Mystery in Space # 61 (Aug., 1960), "Ulthoon" is the name used by an evil scientist who attempts to kill Adam Strange and then wreaks havoc on Rann by using a flying machine to create a devastating tornado.

As Strange eventually deduces, it is the tornado itself which is sentient--the epynonymous Tornado Tyrant--and the evil scientist Ulthoon is just a mental projection created by the Tornado Tyrant to deceive Strange.

I don't know at what point the name Ulthoon was attached to the Tornado Tyrant himself. The latest resource I have available is JLA # 193 (Aug., 1981), when the connexion between the Red Tornado and the Tornado Tyrant was established, and the name "Ulthoon" is not used for the Tyrant, or at all, in that story.

So, at some point after that, some writer--either mistakenly or because it was a logical leap--used "Ulthoon" as the name of the actual Tornado being.
