It wasn't explicitly stated, but they were talking about her actual cousin, not some other Kara's cousin on another Earth.

Wasn't there a hint in early S1 Flash?

Also, it was Eobard's plan that this whole thing was centered around this particular Kara attending the wedding. He clearly has a ton of knowledge about Earth-38.
